Physical Therapist (on-site)
Our client is a healthcare provider dedicated to delivering exceptional care to a rural community in central WI. While situated in a smaller city, the team offers a close-knit and supportive environment with access to post-acute home health and hospital-based services. As a Physical Therapist, you will provide personalized, high-quality care in both outpatient and home health settings, impacting the lives of a diverse range of patients.
Location: Rural healthcare setting with proximity to larger cities.
Schedule: Day shifts, full-time 40 hours per week, with a weekend rotation at the hospital required only once every 10 weeks.

Focus:
Provide physical therapy treatments to patients in both the hospital outpatient setting and post-acute home health.
Develop personalized therapy plans to optimize patient recovery, mobility, and quality of life.
Educate patients and caregivers on continued care and safe physical practices.
Collaborate with a healthcare team dedicated to providing compassionate, effective treatment in a rural setting.
Maintain thorough documentation of patient progress, adhering to regulatory standards for both hospital and home health environments.
Skill Set:
Active Physical Therapist licensure in Wisconsin or the ability to obtain a license before the start date.
Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
Strong communication skills and the ability to build rapport with patients and families in diverse clinical settings.
Efficient time-management and adaptability to care for varying caseloads in both the hospital and home health.
Detail-oriented, with excellent organizational skills for documentation and treatment planning.
