Focus:
• Conduct comprehensive assessments of geriatric patients’ physical, mental, and emotional health, considering medical histories and specific needs.
• Develop individualized care plans in collaboration with the interdisciplinary team for optimal patient outcomes.
• Monitor and document patients’ progress, addressing changes in conditions or needs promptly.
• Provide education and support to patients and families, empowering them in the care process.
• Administer medications to geriatric patients accurately, monitoring for adverse reactions.
• Document medication administration and collaborate with physicians and pharmacists to optimize therapy plans.
• Facilitate seamless transitions of care, coordinating with healthcare providers and community resources.
• Collaborate with the interdisciplinary team to develop discharge plans promoting continuity of care.
• Conduct follow-up assessments post-discharge and adjust care plans as needed.
• Advocate for geriatric patients’ needs, respecting their preferences throughout the care journey.
Skill Set:
• Valid and unrestricted Registered Nurse (RN) license in the state of practice.
• 1 year of experience in a clinical setting, preferably with a focus on geriatric care.
• Knowledge and understanding of the unique physical, cognitive, and psychosocial needs of geriatric patients.
• Excellent assessment and critical thinking skills, with the ability to make sound clinical judgments.
• Strong communication and interpersonal skills, enabling effective collaboration with patients, families, and the interdisciplinary team.
• Proficiency in using electronic health record (EHR) systems and other healthcare technologies.
• Commitment to providing compassionate and patient-centered care.
• Ability to prioritize and manage multiple responsibilities in a fast-paced environment.
• Excellent communication, interpersonal, and organizational skills.
• COVID-19 vaccination and valid driver’s license required.
