Medical Social Worker

Our client is a healthcare group focused on providing top notch medical treatments and care to patients from the comfort of their own homes. They are hiring an experienced Medical Social worker to provide their home health patients with emotional support, care coordination, and connection to resources.

The Medical Social Worker will be responsible for supporting comprehensive clinical social work services for their patients. This individual should have a background in a home health setting and possess prior behavioral science experience.

Focus:

• Perform initial and concurrent psychosocial review of inpatient cases applying evidenced-based criteria.
• Conduct interviews and maintain observation, assessing and tracking progress and patient history.
• Liaise between patients and families/caregivers, connecting them to relevant agencies and resources.
• Work with all disciplines, patients, and families to achieve mutually agreed upon care goals.
• Ensure the highest quality of care is provided and in compliance with federal, state, and local regulatory requirements and established company policies and procedures.
• Identify improvement opportunities and recommend workable solutions.
• Maintain accurate social work documentation, including clinical records and care plans.
• Investigate and resolve patient/family/member concerns regarding patient care as needed.

Skill Set:

• 1+ years of social work experience in healthcare environments. Home health experience preferred.
• Bachelor’s degree in Behavioral Science or Social work.
• Must be a licensed clinical social worker in practicing state. OASIS or other home health certification a plus.
• Necessary soft skills include flexibility, positive attitude, creativity concerning problem solving, and top tier communication skills.
• Valid driver’s license required.
• Current CPR certification required.
